Prescribed opioid induced brain damage in chronic pain patients
Brief description of study
Opioids activate the limbic system and results in acute cell injury hippocampal atrophy. The
specific aims of this proposal are to test the hypotheses that humans with chronic pain on longterm,
high-dose opioids: 1. Will demonstrate smaller hippocampal volumes by MRI. 2. Will
perform more poorly on neuropsychological tests of memory and associated hippocampal
functions. Targeted Population: pts taking chronic opioids for back pain Procedures: MRI and
neuropsychological cognitive tests
Eligibility of study
You may be eligible for this study if you meet the following criteria:

Pain,Opioid

Age: Between 18 Years - 50 Years

Gender: All
Inclusion Criteria:
(1) Chronic lower back pain greater than two years,
(2) PA, NJ , DE residents in the UPenn system for at least 2 years (for PDMP evaluation)
(3) for the opioid group, use of only full mu opioid agonists (morphine, fentanyl, oxycodone) for at least two years with at least 90 MED per day
Exclusion Criteria:
(1) Pregnancy,
(2) MRI contraindications,
(3) Unable to speak/read English,
(4) Comorbid CNS disease that would interfere with informed consent and evaluation of results,
(5) evidence of chronic intake of other confounding drugs (cannabis, benzodiazepines, gabapentin, selective serotonin reuptake inhibitors/antidepressants, pregabalin, illicit drugs) on urine drug tests, PDMP, patient history, EMR.
