Developing a Cultural Adaptation Toolkit to Increase Equity for Underserved Youth with Obsessive-Compulsive and Related Disorders

Improving the quality of evidence-based interventions (EBIs) for pediatric obsessive-compulsive disorder (OCD) and anxiety disorders for underserved youth is a critical step in reducing mental health care inequities. Youth of color and low-income youth have been impacted by systemic and structural inequities (e.g., racism and discrimination, lack of access to needed resources) which have led to disproportionate unmet mental health needs. Improving access to and effectiveness of treatment for these youth, who are underrepresented in clinical trials, is a critical area for OCD research. Despite evidence that community-based clinicians feel that they need to adapt EBIs to better fit the needs of their families, there has been little research to understand the adaptations that are naturally occurring in clinical practice and their effectiveness to reduce psychiatric burden. Moreover, there is a lack of structured guidance for clinicians to make decisions on how to adapt EBIs or what additional strategies may be needed to improve the fit and relevance of services. Clinical research on existing adaptations in community-based practice is needed; this will then facilitate the development of structured guidance for person-centered cultural adaptation
